Qualifications

• Board certification or eligibility in family medicine or internal medicine.
• Experience caring for elderly or frail populations is preferred.
• Active California medical license and DEA registration.

Responsibilities

• Provide primary medical care for seniors, including assessments and care plans.
• Manage chronic diseases and preventive care.
• Coordinate with nurses and specialists.
• Maintain accurate clinical documentation.
