Job summary

Trinity Health in the United States is seeking a Primary Care Physician (PCP) to provide primary medical care to PACE Organization participants. Under the supervision of the Medical Director, you will assess participants, develop plans of care with the interdisciplinary team, and perform medical procedures as indicated.

You will visit nursing home participants, coordinate with specialists, participate in IDT meetings, review outcomes, and help avoid unnecessary hospitalizations.

Qualifications

• MD or DO with active PA license and CDS/DEA as required.
• Board certification in Internal Medicine or Family Practice.
• Geriatrics certification preferred but not required.
• Minimum 1 year experience with frail or elderly populations.
• Strong communication and collaboration with interdisciplinary teams.

Responsibilities

• Provide primary care to PACE participants, assess and plan care.
• Develop and implement care plans with the interdisciplinary team.
• Refer to specialists and coordinate hospital admissions when needed.
• Attend IDT meetings and communicate participant changes.
• Document accurately with appropriate coding and reporting.
