Job summary

Trinity Health Life at Kinder Park is seeking a Primary Care physician to provide comprehensive medical care to PACE participants. You will develop care plans, perform assessments, and coordinate with an interdisciplinary team to optimize health outcomes.

The role requires board certification in Internal Medicine or Family Practice, active licensure, and experience with frail or elderly populations. Travel within the regional centers may be required, and a robust benefits package is offered.

Qualifications

• MD or DO with active license in PO/state.
• CDS Certification and DEA Registration.
• Current BLS certification.
• Board Certification Internal Medicine or Family Practice; geriatrics preferred.
• Minimum 1 year working with frail or elderly population.
• Valid driver’s license and transportation.
• Ability to work with minimal supervision and in a virtual team.
• Strong communication and critical thinking skills.
• Willingness to travel within the geographic region.

Responsibilities

• Provide primary medical care to PACE participants with team-developed care plans.
• Assess, plan, implement, and evaluate care in collaboration with the interdisciplinary team.
• Refer to specialists; manage in nursing home and hospital admissions when needed.
• Oversee use of medical services and follow up with specialists.
• Attend interdisciplinary team meetings and communicate patient changes.
• Review outcomes data to improve practices and reduce hospitalizations.
• Complete documentation and diagnostic coding.
• Assist leadership with policy development and staff training.
