Job summary

RSC Healthcare LLC is seeking a Primary Care Provider to serve participants at a PACE Center under the direction of the Regional Medical Director. The role involves medical assessment, care-plan development with the IDT, and ongoing management of acute and chronic conditions.

Responsibilities include conducting reassessments, triaging changes, and educating patients and families. Requires board certification in Internal Medicine or Family Practice, Indiana license, and geriatrics experience.

Qualifications

• Must have medical degree and board certification in Internal Medicine or Family Practice.
• Licensed to practice medicine in Indiana.
• Experience with geriatrics and long-term care settings preferred.
• CPR and DEA certifications up to date.

Responsibilities

• Provide primary care within a PACE Center under Regional Medical Director.
• Develop and implement patient care plans with the IDT.
• Conduct medical assessments and reassessments as required.
• Educate patients and families about health maintenance and chronic conditions.
