Hartford HealthCare invites Nurse Practitioners and Physician Assistants to join our geriatric outpatient team at Seabury Active Life Plan Community in Bloomfield, CT. This role supports 100% outpatient geriatric primary care and consults, with a flexible schedule offering part-time or full-time options on weekdays.

Qualifications

• CT licensure (or eligible) to practice in CT.
• APRN (Family/Adult/Geri) or PA credential required.
• Experience in geriatric or primary care settings preferred.

Responsibilities

• Perform routine physical examinations and develop medical, educational, and psychosocial plans.
• Coordinate with care team for further evaluation and referrals as needed.
• Provide geriatric consults and manage an individual primary care panel.
