Hartford HealthCare in Windsor, CT is seeking a Nurse Practitioner or Physician Assistant for 100% outpatient Family Medicine. The role supports a collaborative team including physicians, an embedded behavioral health provider, and a clinical pharmacist to deliver patient-centered care.

Open to FNPs and PAs with 1+ year of primary care experience. Full-time, weekdays only, with no on-call obligations. Hartford HealthCare promotes development and integrated care across Connecticut.

Qualifications

• Licensure: Unrestricted State of Connecticut Medical License (or eligible).
• Training: APRN (Family Nurse Practitioner) or PA
• Experience: 1+ years’ experience in a primary care or relevant outpatient setting.

Responsibilities

• Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients.
• Develop plans for further evaluation in consultation with care team.
• Provide treatment within scope and refer outside when needed.
