Job summary

HSHS Medical Group is recruiting a Board Certified Nurse Practitioner or Physician Assistant to join our Geriatric Primary Care team in Springfield, IL. This 100% outpatient practice emphasizes comprehensive geriatric care with a Monday–Friday schedule and no holidays or weekends, supported by EPIC EMR.

Base salary starts at $140,481 with bonus potential, and a robust benefits package including CME, 401(k) retirement plan, and medical, dental and vision coverage.

Qualifications

• Unrestricted Illinois license prior to start.
• Board Certified as either an APRN or PA.
• Primary Care/Family Medicine experience strongly preferred, but not required.

Responsibilities

• Provide geriatric primary care in outpatient setting.
• Collaborate with multidisciplinary team to coordinate care.
• Document patient encounters in EPIC EMR.
• Support care coordination and follow-up for complex patients.
