Job summary

Curana Health is seeking an experienced Psychiatric Nurse Practitioner (PMHNP) to deliver high-quality geriatric behavioral health care across SNF, LTC, AL, and IL settings. You will join a multidisciplinary team to provide personalized psychiatric services and ensure continuity of care throughout post-acute and long-term care.

The role requires a PMHNP, ANCC certification, active DEA license, and state NP licensure, with travel to facilities.

Qualifications

• Graduation from an accredited Nurse Practitioner program and ANCC certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP).
• Active DEA license.
• BLS and ACLS certification.
• Current Nurse Practitioner license in the state of practice.

Responsibilities

• Psychiatric Care and Rounding: Conduct patient rounds and manage psychiatric care within SNF, LTC, AL, and IL settings.
• Collaborative Care Coordination: Partner with attending physicians, consulting specialists, referral sources, and facility staff to ensure comprehensive, coordinated care.
• Patient Evaluation and Management: Assess and manage the mental health needs of residents, perform physical exams, and gather complete medical histories.
• Diagnostics and Therapeutics: Order and interpret lab and diagnostic tests, make preliminary diagnoses, and develop effective therapeutic care plans for mental and physical health.
• Medication and Treatment Management: Prescribe and administer medications and treatments; provide injections as necessary.
• Patient and Family Education: Educate patients and families on medications, care plans, and preventive mental health practices.
• Acute Care Collaboration: Work alongside physicians and facility teams to address acute psychiatric and medical needs promptly and effectively.

Education
