Job summary

Total Care Therapy is hiring a Physical Therapist to join our therapist-owned team in Columbus, OH. You will evaluate residents, design and implement treatment plans, and help improve mobility and independence for older adults in assisted living and independent living communities.

You will deliver evidence-based therapies, reassess progress, and document in our EMR system. A driver’s license and CPR/BLS are required. We value collaboration, mentorship, and opportunities for growth.

Qualifications

• Licensed Physical Therapist in the state where you practice.
• CPR/BLS certification current and valid.
• Driver's license and reliable transportation required.

Responsibilities

• Evaluate residents and develop individualized care plans.
• Provide skilled PT treatments including gait, balance and mobility training.
• Reassess progress and modify treatment plans accordingly.
• Supervise PTAs per state regulations.
• Document timely in EMR and educate residents and caregivers.
• Collaborate with interdisciplinary team for patient-centered care.
• Maintain an active clinical caseload while delivering therapy.
