Job summary

Total Care Therapy LLC in Westerville, OH is seeking a compassionate Physical Therapist to evaluate, develop, and implement individualized treatment plans that help residents improve mobility, restore function, reduce pain, and maintain independence.

In this full-time, on-site role you will deliver skilled PT services, supervise PTAs, complete timely EMR documentation, educate residents and staff, and collaborate with an interdisciplinary team across assisted living and independent living

Qualifications

Current State Physical Therapist (PT) License.
CPR/BLS Certification required.
Valid driver's license and reliable transportation.
Experience in geriatrics/home health/assisted living preferred.

Responsibilities

Evaluate residents and develop individualized plans of care.
Provide skilled physical therapy treatments, including exercise and mobility training.
Reassess progress and modify treatment plans as appropriate.
Supervise PTAs in accordance with state regulations.
Document using the EMR in a timely and accurate manner.
Educate residents, caregivers, and staff on treatment plans and home programs.
Collaborate with interdisciplinary team for coordinated care.
Maintain a productive clinical caseload while delivering quality therapy.
