Job summary

Total Care Therapy is seeking a compassionate Physical Therapist to join our growing team serving assisted living and independent living communities in Ohio. You will evaluate residents, create individualized treatment plans, and deliver evidence-based PT to improve mobility and independence.

Qualifications

• Current State Physical Therapist License in Ohio is required.
• Experience in geriatrics, home health, assisted living, or skilled nursing is preferred but not required.
• CPR/BLS certification is required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments including exercise, gait, balance and mobility training.
• Reassess progress and modify treatment plans as needed.
• Supervise PTAs in accordance with state regulations.
• Document using EMR in a timely and accurate manner.
• Educate residents, caregivers and staff on treatment plans and fall prevention.
• Collaborate with interdisciplinary team for coordinated patient care.
• Maintain a productive clinical caseload delivering high-quality therapy.

About the Role

We are seeking a compassionate and motivated Physical Therapist to join our growing team. In this role, you will evaluate, develop, and implement individualized treatment plans that help residents improve mobility, restore function, reduce pain, and maintain independence.

Responsibilities

• Evaluate residents and develop individualized plans of care.

• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.

• Reassess patient progress and modify treatment plans as appropriate.

• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.

• Complete timely and accurate documentation using the electronic medical record (EMR).

• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.

• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.

• Maintain a productive clinical caseload while delivering exceptional therapy services.

Qualifications

• Current State Physical Therapist (PT) License

• Current CPR/BLS Certification

• Valid driver's license, reliable transportation, and auto insurance

• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
