Job summary

Total Care Therapy LLC seeks a compassionate Physical Therapist to join our growing team in Shelby Township, MI. You will evaluate, develop, and implement individualized treatment plans to improve mobility, restore function, reduce pain, and maintain independence for residents in assisted living and independent living communities.

Responsibilities include supervising PTAs, documenting in EMR, educating residents and staff, and collaborating with an interdisciplinary team.

Qualifications

• Current State Physical Therapist (PT) license.
• CPR/BLS certification.
• Valid driver's license and reliable transportation.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
