Total Care Therapy in Shelby Township, MI is seeking a compassionate Physical Therapist to evaluate, develop, and implement individualized treatment plans for residents in assisted living and independent living communities. You will promote mobility, independence, and quality of life while working with an interdisciplinary team.

Formación

• Current State Physical Therapist license.
• CPR/BLS Certification.
• Valid driver’s license with reliable transportation.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsabilidades

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess progress and modify treatment plans as needed.
• Supervise PTAs in accordance with state regulations.
• Complete timely documentation using EMR.
• Educate residents, caregivers, and staff on treatment plans, home exercises, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team for coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.
