Majestic Care is seeking a licensed Physical Therapist to join our skilled nursing facility team, focusing on evaluating and treating residents to improve mobility, strength, and independence. You will design individualized therapy plans, lead interventions, and collaborate with an interdisciplinary team to deliver resident-centered care.

Qualificações

• Current state licensure as a Physical Therapist.

Responsabilidades

• Conduct initial and ongoing evaluations to assess residents’ physical therapy needs.
• Develop and implement individualized treatment plans based on clinical findings and goals.
• Provide direct physical therapy interventions including exercises, gait and balance training, mobility enhancement, and manual therapy.
• Utilize evidence-based practices to promote functional outcomes.
• Monitor and document residents’ progress and adjust plans as needed.
• Educate residents, families, and caregivers on treatment plans and home exercises.
• Collaborate with nursing, occupational therapy, and speech therapy for coordinated care.
• Maintain accurate documentation of all evaluations, treatments, progress notes, and discharge planning.
• Ensure compliance with HIPAA, OSHA, infection control, and PDPM requirements.
• Stay current with advancements in PT and geriatric rehab; pursue continuing education.
• Provide training to PTAs and team members to ensure clinical excellence.
• Oversee PTAs per state acts; offer coaching and corrective actions.
• Evaluate team performance and provide constructive feedback.
