Job summary

Sweetwater Care in Perrysburg, Ohio seeks a compassionate Physical Therapist to provide rehabilitative therapy in a skilled nursing setting, helping seniors regain function and quality of life. You will work under regulatory standards and with the rehab team.

The role reports to the Director of Rehabilitation Services and offers full-time, part-time, or PRN schedules. Pay is dependent on experience, with benefits including health insurance, PTO, and professional development opportunities.

Qualifications

• Bachelor's degree or higher in Physical Therapy or related field.
• Active state license.
• 2+ years of experience in a skilled nursing facility environment.

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL physical functioning disorders and develop an appropriate plan of care.
• Utilize standardized assessments and evidence-based practice to support clinical interventions.
• Provide rehabilitative, skilled treatment per regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan (ADL retraining, therapeutic activities, exercises, modalities, assistive devices).
• Assess progress and adjust treatments in the clinical record per regulatory requirements.
• Evaluate treatment outcomes and modify services accordingly.
• Provide discharge summaries of services per regulatory requirements.
• Document all regulatory and clinical information in the electronic system and maintain patient records.
