Job summary

Sweetwater Care in Scenic Heights, TN is seeking a compassionate Physical Therapist to provide rehabilitative therapy in a skilled nursing environment. You will work with seniors to improve function and quality of life, following regulatory and clinical practice standards.

The role reports to Rehabilitation Director and involves patient evaluation, treatment planning, documentation in the electronic system, and collaboration with the care team.

Qualifications

• Bachelor's degree or higher in Physical Therapy or related field.
• Active state license.

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL physical functioning disorders and develop an appropriate plan of care.
• Utilize standardized assessments and evidence-based practice to support interventions.
• Provide rehabilitative, skilled and medically necessary treatment per regulatory requirements.
• Document treatment minutes and progress in the electronic documentation system.
• Communicate with supervisors and health team members about progress and plans.
• Prepare discharge summaries per clinical requirements.
