Job summary

Sweetwater Care in Kent, OH is seeking a compassionate Physical Therapist to provide rehabilitative therapy in a skilled nursing setting, guiding patients through cognitive and ADL recovery and striving to restore independence. You will collaborate with a multidisciplinary team, document progress, mentor assistants, and ensure compliant, patient-centered care.

Must hold an active state license and a degree in Physical Therapy with at least two years in SNF, with full-time/part-time/PRN

Qualifications

• Bachelor's degree in Physical Therapy or related field required.
• Active state license required.
• Minimum 2 years of experience in a skilled nursing facility.

Responsibilities

• Screen/evaluate patients with cognitive/ADL disorders and plan care.
• Apply evidence-based rehab interventions and document progress.
• Provide rehabilitative, skilled treatment per regulatory requirements.
• Supervise PT assistants and rehab aides as needed.
• Communicate progress with care team and families.
• Complete discharge summaries and maintain records per policy.
