Job summary

BoldAge PACE is hiring a Physical Therapist to plan and deliver comprehensive PT services for participants in the PACE program. The role involves assessments at enrollment, developing treatment plans, and collaborating with the interdisciplinary team to support community-based care.

You will educate participants and families, participate in staff training, and ensure documentation meets policy standards while promoting safety and independence for seniors at home or in centers.

Qualifications

• Advanced degree in Physical Therapy from an accredited school.
• Current license to practice Physical Therapy in the state.
• Current driver’s license and proof of auto insurance.
• Two years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable.
• 1 year of experience with a frail or elderly population is preferred or training provided on hire.

Responsibilities

• Plans, organizes, and implements Physical Therapy services for PACE participants.
• Performs in-person assessments for participants at initial enrollment and ongoing assessments as determined by the IDT.
• Develops and maintains treatment plans and documents evaluation, plan and treatment.
• Educates participants and families on home programs and safety.
• Trains PACE staff in body mechanics, equipment use, transfers, gait patterns, and ADLs.

Education

Advanced degree in Physical Therapy

Current license to practice Physical Therapy in the state

Current driver's license and proof of auto insurance

Two years of experience on an interdisciplinary team in hospital/nursing home/community setting (preferable)
