Job summary

The PT collaborates with nursing, occupational, and speech therapists to ensure resident-centered care, maintain documentation per regulations, and participate in quality improvement. Current state licensure required by state.

• Current state licensure as a Physical Therapist.

Responsibilities

• Evaluate residents to assess PT needs and develop treatment plans.
• Provide PT interventions: therapeutic exercises, gait and balance training, and manual therapy.
• Document progress and adjust plans per changes.
• Educate residents and families on home exercises and safety.
