Job summary

The Chatham Hills Subacute Care Center is seeking an Experienced PT for our Facility. The Physical Therapist will assess referred patients requiring rehabilitation, establish treatment programs, and document patient progress while educating families and patients.

The role involves supervising Physical Therapy Assistants, collaborating with all disciplines, and ensuring exemplary patient service and clinical excellence in a long-term care setting.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Oversees and supervises Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred.
• Well versed with documentation writing.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence.
• Report to work on time and adhere to scheduled hours with professionalism.
• Perform all other duties assigned by Director of Rehabilitation.
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
• Experience with Case Mix Index and PPS scheduling is preferred.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals matching patient capacity and tolerance.
• Document and record patient conditions and educate patients and families in physical therapy methods.
• Oversee daily operations of the Physical Therapy department.
• Collaborate with other disciplines to deliver patient-centered care.
