Job summary

Chatham Hills Subacute Care Center is seeking an Experienced Physical Therapist (PT) to perform comprehensive rehabilitation services for our residents. The PT will assess patients, implement individualized treatment plans, and educate families on therapy approaches within a multidisciplinary team.

The role includes supervising Physical Therapy Assistants, maintaining detailed documentation, and collaborating with physicians to maximize patient outcomes in a long-term care setting.

Qualifications

• Graduate from an accredited Physical Therapy program.
• Must hold a valid state license and registration.
• Experience supervising Physical Therapy Assistants is required.
• Experience with long-term/geriatric care is preferred.
• Strong documentation skills are essential.
• Experience with Case Mix Index and PPS scheduling is preferred.

Responsibilities

• Assess referred patients and provide direct rehabilitation care.
• Develop and administer treatment programs with patient-specific goals.
• Educate patients and families on physical therapy methods.
• Document patient condition and progress; record weekly notes and discharges.
• Oversee and supervise Physical Therapy Assistants.
• Collaborate with other disciplines to provide patient-centered care.
• Maintain confidentiality and HIPAA compliance.
