Job summary

Chatham Hills Subacute Care Center is seeking an experienced Physical Therapist to assess and treat patients requiring rehabilitation in a subacute care setting. You will establish treatment plans, document progress, and educate patients and families under physician direction.

The ideal candidate will have a PT degree, a valid state license, and experience in long-term or geriatric care, with strong documentation and teamwork skills.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Oversees and supervises Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred.
• Well versed with documentation writing.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide patient-centered care.
• Exceptional patient service skills and clinical excellence.
• Punctuality and professionalism in scheduling.
• Performs duties as directed by Director of Rehabilitation.
• Confidentiality and HIPAA compliance; respect residents' rights.
• Experience with Case Mix Index and PPS scheduling is preferred.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals based on patient capacity.
• Set realistic goals, document patient condition, and educate patients and families.
• Coordinate care with physicians, nurses, and other disciplines to ensure optimal outcomes.
