Job summary

The Chatham Hills Subacute Care Center is seeking an Experienced Physical Therapist (PT) for our facility. The PT will assess referred patients requiring rehabilitation, develop treatment plans, and provide direct patient care in a geriatric long-term care setting.

The role requires a licensed PT in the state, graduation from an accredited PT program, and experience supervising Physical Therapy Assistants, with strong documentation and multidisciplinary collaboration.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Must hold a valid PT license/registration for state of practice.
• Oversee and supervise Physical Therapy Assistants.
• Experience in long-term care and geriatric settings preferred.
• Strong documentation and record-keeping skills.
• Collaborate with all disciplines for patient-centered care.

Responsibilities

• Assess referred patients requiring rehabilitation and determine baselines.
• Establish and administer treatment programs with goals aligned to capacity and tolerance.
• Document patient conditions and progress; educate patients and families.
• Coordinate with physicians and care teams to optimize outcomes.
• Supervise and guide Physical Therapy Assistants; ensure compliance and quality.
