Job summary

Chatham Hills Subacute Care Center is seeking an experienced Physical Therapist to assess and treat patients requiring rehabilitation. The PT will develop individualized treatment plans, document progress, and educate patients and families under physician direction.

The role includes supervising Physical Therapy Assistants, collaborating with all disciplines, and ensuring HIPAA compliance. Long-term care or geriatric experience is preferred, with strong documentation and professionalism a must.

Qualifications

• Graduate from an accredited Physical Therapy program.
• Valid state license and registration.
• Experience supervising Physical Therapy Assistants.
• Experience in long-term/geriatric care preferred.
• Strong documentation skills.
• Experience with evaluations, progress notes, and discharge documentation.
• Ability to collaborate with all disciplines.
• Professionalism and patient service orientation.
• HIPAA confidentiality compliance.
• Familiarity with Case Mix Index and PPS scheduling preferred.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer a treatment program with goals based on patient capacity and tolerance under physician direction.
• Document patient condition and educate patients and families in appropriate physical therapy methods.
• Set realistic and achievable goals for patients.
• Oversee and supervise Physical Therapy Assistants.
• Collaborate with all disciplines to provide patient-centered care.
• Maintain confidentiality and HIPAA compliance.
• Experience with Case Mix Index and PPS scheduling preferred.
