Job summary

The Chatham Hills Subacute Care Center is seeking an experienced Physical Therapist to join our rehabilitation team. The PT will assess referred patients, develop and oversee treatment plans, and coordinate with physicians to optimize functional outcomes for residents.

You will supervise Physical Therapy Assistants, ensure accurate documentation, and educate patients and families. A valid PT license and accreditation are required, with preferred experience in long-term or geriatric care.

Qualifications

• Must graduate from an accredited Physical Therapy program.
• Must hold a valid license/registration to practice.
• Experience with long-term care and geriatric care preferred.
• Experience with case mix index and PPS scheduling is preferred.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals per patient capacity.
• Document patient condition and educate patients and families in PT methods.
• Oversee and supervise Physical Therapy Assistants and collaborate with all disciplines to provide patient-centered care.
