Job summary

Chatham Hills Subacute Care Center is seeking an Experienced Physical Therapist (PT) to join our facility. The PT will assess referred patients, develop and supervise rehabilitation programs, and document patient progress under physician direction.

The ideal candidate will have an accredited PT degree, valid license, and experience with long-term/geriatric care, plus strong documentation skills and teamwork across disciplines.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Overseeing and supervising Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred.
• Well versed with documentation writing.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence.
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
• Experience with Case Mix Index and PPS scheduling is preferred however not limited to.

Responsibilities

• Assess referred patients and provide direct patient care.
• Establish and administer treatment programs with goals based on patient capacity and tolerance.
• Set realistic goals, document patient condition and progress.
• Educate patients and families on physical therapy methods.
• Oversee and supervise Physical Therapy Assistants.
• Collaborate with all disciplines to provide patient-centered care.
• Maintain confidentiality and HIPAA compliance.
• Document evaluations, progress notes, recertifications and discharge plans.
• Arrive on time and adhere to scheduled hours.
