Job summary

Chatham Hills Subacute Care Center is seeking an experienced Physical Therapist (PT) to join our facility. The PT will assess referred patients, develop and administer a rehab program, document progress, and educate patients and families under physician direction.

The role requires an accredited PT degree, active state licensure, and experience with geriatric care. The incumbent will supervise PTAs and collaborate with multidisciplinary teams to ensure high-quality patient care.

Qualifications

• Graduated from an accredited Physical Therapy college or university.
• Must hold a valid professional license and registration for state of practice.
• Overseeing and supervising Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Well versed with documentation writing and timely progress notes.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer a treatment program with goals based on patient capacity.
• Document patient condition and educate patients and families in therapy methods.
• Collaborate with all disciplines to provide patient-centered care.
• Adhere to scheduled hours and maintain professional conduct.
