Qualifications

Position Summary: As a Physical Therapist in the PACE program, you will evaluate participants, administer physical therapy treatments, track progress, and provide instruction to participants and their families on care practices. You will also supervise aides, assistants, and students, working both in the PACE Center and in participants’ homes. Responsibilities include conducting initial and periodic assessments, developing personalized care plans, coordinating 24-hour care delivery, and collaborating with an interdisciplinary team (IDT) to ensure the best care. You’ll stay vigilant to the medical, functional, and psychosocial needs of each participant, documenting all changes in accordance with our policies.

Requirements:

  • Bachelor's degree in Physical Therapy (if obtained before 2000) or Master’s degree in Physical Therapy (if obtained after 2000).
  • Knowledge and clinical competency working with the geriatric population.
  • Current Physical Therapist license in the state of practice.
  • Completion of PACE-specific competency requirements before working independently.
  • 1 year of experience working with frail or elderly populations preferred (training provided if not present).
  • Reliable transportation, valid driver’s license, and minimum state-required liability auto insurance.
  • Medical clearance for communicable diseases and up-to-date immunizations prior to direct participant contact.

Schedule:

  • Monday – Friday, 8:00 AM – 4:30 PM
  • 40 hours per week
