Job summary

BoldAge PACE seeks a Physical Therapist to plan, organize, and implement PT services for participants. You will assess, plan, treat, and educate participants, caregivers, and families to maintain community-based support.

Join a team-focused program delivering comprehensive care with a focus on independence and quality of life for older adults. Relevant PT license and experience with seniors preferred.

Qualifications

• Advanced degree in Physical Therapy
• Current license to practice Physical Therapy in the state
• Current driver’s license and proof of auto insurance
• Two years of inter-disciplinary team experience preferred
• One year with frail or elderly population preferred

Responsibilities

• Provide high-quality PT care as part of the IDT.
• Perform initial and ongoing in-person assessments.
• Assess home safety and equipment needs; recommend mods.
• Plan and implement PT treatment programs per standards.
• Document evaluation, plan, and treatment accurately.
• Educate participants and families on home programs.
• Train staff in body mechanics and safety.
• Collaborate on a comprehensive care plan for participants.
