Job summary

Water’s Edge Rehab and Nursing Center at Port Jefferson is seeking an Experienced Physical Therapist (PT) for our facility. The role includes direct patient care, assessment, and development of individualized treatment plans under physician direction.

The candidate should be a licensed PT with experience in geriatric care, documentation, and collaboration across disciplines to deliver patient-centered therapy and ensure compliance with HIPAA and resident rights.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Compliance with confidentiality, HIPAA and Federal Residents Rights.

Responsibilities

• The Physical Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate physical therapy method.
• Oversee and supervise Physical Therapy Assistants.
