Job summary

Sweetwater Care in Ohio is seeking an Occupational Therapist to join our skilled nursing team, delivering rehabilitative therapy focused on cognitive and ADL function to residents. You will assess, plan, implement, and document care while coordinating with the rehab director and clinical staff.

Ideal candidates have a Bachelor's or higher in Occupational Therapy, an active Ohio license, and at least 1 year in SNF settings.

Qualifications

• Bachelor's degree or higher in Occupational Therapy.
• Active Ohio state license.
• 1+ year of experience in a skilled nursing facility.
• Ability to supervise PTAs and rehab aides.

Responsibilities

• Screen/evaluate patients with cognitive/ADL disorders and develop care plans.
• Utilize standardized assessments to support interventions.
• Provide rehabilitative therapy and ensure regulatory compliance.
• Document treatment plans and progress in medical records.
• Communicate progress and discharge plans with the care team.
• Collaborate with supervisors and health team members.
