Job summary

Therapy Management Corporation (TMC) is seeking a Physical Therapist to empower adult/geriatric patients through compassionate, patient-centered therapy. You’ll evaluate and treat adults in skilled nursing and long-term care settings, focusing on independence, mobility, safety, and quality of life while collaborating with nurses and other clinicians.

Qualifications

• Must be licensed PT or license eligible in the state of practice.
• Graduate of an accredited Physical Therapist program.
• Interest or experience in skilled nursing or geriatric rehabilitation.

Responsibilities

• Evaluate patients and provide personalized physical therapy treatments that help patients improve mobility, strength, balance, and functional independence.
• Develop and implement patient-centered treatment plans focused on long-term rehabilitation goals.
• Deliver hands-on therapy interventions designed to support recovery, increased endurance, and enhance overall quality of life in older adults.
• Help patients restore functional mobility and safely perform daily activities with greater confidence and independence.
• Partner with interdisciplinary teams, caregivers, and nursing staff to drive meaningful patient outcomes and continuity of care.
• Track patient progress, maintain accurate clinical documentation, and adapt treatment plans based on patient needs and rehabilitation goals.
