Job summary

Therapy Management Corporation (TMC) is seeking a licensed Physical Therapist to empower adult and geriatric patients through compassionate, patient-centered care. The PT will evaluate and treat adults in skilled nursing and geriatric rehab settings, focusing on independence and quality of life.

Qualifications

• Licensed PT or license eligible in state of practice.
• Graduate of an accredited Physical Therapist Program.
• Interest or experience in skilled nursing, long-term care or geriatric rehabilitation.
• Passion for patient-centered care and improving quality of life for older adults.

Responsibilities

• Evaluate patients and provide personalized physical therapy treatments to improve mobility, strength, balance and functional independence.
• Develop and implement patient-centered treatment plans focused on long-term rehabilitation goals.
• Deliver hands-on therapy interventions to support recovery and functional outcomes in older adults.
• Help patients restore functional mobility and safely perform daily activities with greater confidence.
• Partner with interdisciplinary teams, caregivers and nursing staff to drive meaningful patient outcomes and continuity of care.
• Track patient progress, maintain accurate clinical documentation and adapt plans as needed.
