Therapy Management Corporation (TMC) is seeking a licensed Physical Therapist to provide patient-centered care to adults and geriatric populations in Phoenix, AZ. You will assess mobility and functional goals, and work within a collaborative clinical environment to deliver quality outcomes.

You will engage in interdisciplinary teamwork, maintain thorough documentation, and contribute to a care plan focused on independence and improved quality of life for older adults.

Qualifications

• Licensed Physical Therapist (PT) in the state of practice.
• Graduate of an accredited Physical Therapist Program.
• Interest or experience in skilled nursing, long‑term care or geriatric rehabilitation.
• Passion for patient‑centered care and improving quality of life for older adults.

Responsibilities

• Evaluate patients and provide personalized physical therapy treatments to improve mobility, strength, balance, and independence.
• Develop and implement patient-centered treatment plans with long-term rehabilitation goals.
• Deliver hands-on therapy interventions to support recovery and endurance in older adults.
• Help patients restore functional mobility and safely perform daily activities with confidence.
• Collaborate with interdisciplinary teams, caregivers, and nursing staff to drive patient outcomes and continuity of care.
• Track patient progress, maintain accurate clinical documentation, and adapt plans as needed.
