Job summary

Therapy Management Corporation (TMC) is seeking a Licensed Physical Therapist to empower adults and seniors with compassionate, patient-centered therapy. You will deliver PT services to adult/geriatric populations, driving independence, mobility, safety and quality of life in a collaborative clinical environment.

Qualifications

• Licensed Physical Therapist (PT) or license eligible in the state of practice.
• Graduate of an accredited Physical Therapist Program.
• Interest or experience in skilled nursing, long-term care or geriatric rehabilitation.
• Passion for patient-centered care and improving quality of life for older adults.

Responsibilities

• Evaluate patients and provide personalized physical therapy treatments to improve mobility, strength, balance and functional independence.
• Develop and implement patient-centered treatment plans focused on long-term rehabilitation goals.
• Deliver hands-on therapy interventions to support recovery, endurance and quality of life in older adults.
• Help patients restore functional mobility and safely perform daily activities with greater confidence.
• Partner with interdisciplinary teams, caregivers and nursing staff to drive meaningful outcomes and continuity of care.
• Track patient progress, maintain accurate clinical documentation, and adapt plans as needed.
