Job summary

BoldAge PACE in South Carolina is seeking a Physical Therapist to join our all-inclusive program of care for seniors. You will assess, plan, and deliver physical therapy services to participants, educating families and collaborating with the interdisciplinary team to promote independence in the community.

Responsibilities span assessments, treatment plans, adaptive equipment coordination, home safety evaluations, and ongoing documentation in medical records.

Qualifications

• Advanced degree in Physical Therapy from an accredited school.
• Current license to practice Physical Therapy in the state.
• Current driver's license and proof of auto insurance.
• Two (2) years of experience on an interdisciplinary team in a hospital, nursing home or community-based setting preferred.
• 1 year of experience with a frail or elderly population preferred.

Responsibilities

• Provides high quality care as a member of the PACE interdisciplinary team (IDT).
• Performs in-person assessments at initial enrollment and ongoing as determined by the IDT.
• Plans physical therapy treatment programs according to standards and policies.
• Administers therapy treatment and educates participants and families.
• Maintains documentation of clinical service in medical records.
• Collaborates with the IDT to develop a comprehensive care plan.
• Participates in quality improvement projects and meetings.

Education

Advanced degree in Physical Therapy

Current license to practice Physical Therapy in the state
