Job summary

Nurses and More, Inc. seeks a Physical Therapist (PT) for home health care to deliver one-on-one rehabilitation in patients’ homes, improving mobility and independence. The role runs Monday–Friday, 8 AM–5 PM in Ladson, SC, and requires home care experience and active SC licensure.

Pay is $53/hour with benefits that include weekly pay and PTO. Candidates should have a valid driver’s license, BLS certification, and reliable transportation for in-home visits.

Qualifications

• Graduate from an accredited school of Physical Therapy.
• Licensed Physical Therapist in the State of SC.
• Minimum 1 year of experience.
• Valid driver's license.
• AHA BLS Provider certification.
• Reliable transportation.
• Home care experience.

Responsibilities

• Provides direct hands-on patient care following the plan of care and current standards of practice for Physical Therapy.
• Documents all care provided, coordination of services, orders, goals, etc. in the patient’s electronic medical record in an accurate and timely manner.
• Supervises care provided by the PTA and Home Health Aides to ensure that treatment goals are being met.
• Coordinates with other disciplines, agencies/companies, families, etc. to ensure that the care needs of the patient are met.
