Job summary

Good Shepherd Lutheran Community in Blair, NE is seeking a compassionate Physical Therapist (PT) to join the rehabilitation team in a Full-Time or Per Diem capacity. The PT will help residents improve mobility, strength, balance, and functional independence in a resident-centered care setting.

Qualifications

• DPT or Bachelor/Master's in Physical Therapy from an accredited program.
• Nebraska Physical Therapist license in good standing.

Responsibilities

• Evaluate residents' physical function, mobility, strength, balance, endurance, and functional limitations.
• Develop individualized physical therapy treatment plans based on each resident's clinical needs, goals, and level of function.
• Provide skilled interventions to improve strength, balance, coordination, endurance, mobility, and functional independence.
• Perform gait training, transfer training, therapeutic exercises, neuromuscular re-education, and functional mobility training.
• Assess and recommend assistive devices and mobility strategies.
• Educate residents and caregivers on exercises, mobility techniques, fall prevention, safety, and home recommendations.
• Monitor progress and modify treatment plans as needed.
• Collaborate with physicians, nursing, OT, SLP, and other team members.
• Participate in care planning, discharge planning, and clinical activities.
