Good Shepherd Lutheran Community in Blair, NE is seeking a compassionate Physical Therapist (PT) to join the rehabilitation team in full-time or per diem capacity. You will work closely with residents, families, nursing staff, and physicians to provide resident-centered care.

Qualifications include a DPT/PT degree and Nebraska PT license. New graduates are welcome. The role emphasizes improving mobility, independence, and quality of life in a supportive, interdisciplinary setting.

Qualifications

• DPT or PT degree from an accredited program.
• Current Nebraska Physical Therapist license in good standing.
• Strong clinical reasoning and communication skills.
• Ability to work effectively with older adults and varying levels of ability.
• Experience in skilled nursing, senior living, rehab, geriatrics (preferred but not required).
• New graduates with appropriate licensure are welcome to apply.
• Commitment to compassionate, resident-centered care.
• Ability to work independently while collaborating with an interdisciplinary team.

Responsibilities

• Evaluate residents' physical function, mobility, strength, balance, endurance, and functional limitations.
• Develop individualized physical therapy treatment plans based on clinical needs, goals, and function.
• Provide skilled interventions to improve strength, balance, endurance, mobility, and independence.
• Perform gait, transfer training, therapeutic exercises, and functional mobility training.
• Assess and recommend assistive devices and mobility strategies.
• Educate residents, families, and caregivers on exercises, safety, and home programs.
• Monitor progress and modify treatment plans as needed.
• Collaborate with physicians, nursing, and other rehab professionals.
• Participate in care planning, discharge planning, and interdisciplinary meetings.
• Maintain accurate clinical documentation per policies and regulations.
• Promote safety, dignity, independence, and quality of life.
