Job summary

Majestic Care of West Allen is seeking a licensed Physical Therapist to join our skilled nursing facility team and deliver resident-centered care. You will evaluate residents, develop treatment plans, and provide therapeutic interventions to improve mobility, balance, and independence.

Collaborating with nursing, OT, and speech therapy, you will educate families, document progress, and ensure compliance with state and federal regulations while advancing geriatric rehabilitation goals.

Qualifications

• Doctorate or Master’s degree in Physical Therapy from an accredited program.
• Current state licensure as a Physical Therapist.

Responsibilities

• Conduct initial and ongoing evaluations of residents' physical therapy needs, including mobility, strength, balance, and functional limitations.
• Develop and implement individualized treatment plans based on clinical findings, resident goals, and functional abilities.
• Provide direct physical therapy interventions such as therapeutic exercises, gait and balance training, mobility enhancement, and manual therapy.
• Utilize evidence-based practices to promote optimal functional outcomes and independence.
• Monitor and document residents’ progress, adjusting treatment plans as necessary to reflect changes in condition or goals.
• Educate residents, families, and caregivers on treatment plans, home exercises, and safety strategies to support continued progress and prevent injury.
• Collaborate with interdisciplinary team members—including nursing, occupational therapy, and speech therapy—to ensure coordinated, resident-centered care.
• Oversee and support physical therapy assistants (PTAs) in accordance with state practice acts and facility guidelines.
• Provide coaching, development, and corrective action as needed to ensure high-quality service delivery.
