Job summary

Fortis Care's physiatry Advanced Practice Provider sees residents at our Savannah partner SNF, addressing mobility barriers with targeted injections and spasticity management. You’ll partner with PT/OT, nursing, and the attending team to restore function and enable therapy participation.

You’ll own a resident panel and advance care plans, coordinating DME and adaptive equipment while ensuring smooth handoffs back to therapy and primary teams.

Qualifications

• Licensed NP or PA in Georgia.
• NPs: board certification in Internal Medicine, Geriatrics, or Family Practice.
• PAs: NCCPA certification required.

Responsibilities

• Assess residents unable to progress with focused exams to identify barriers.
• Provide direct treatment: injections (knee, shoulder, hip), spasticity management, trigger point/dry needling, meds as needed.
• Collaborate with PT/OT, nursing, and attending to plan DME, bracing, and adaptive equipment.
