Job summary
Trio Health Partners is seeking a Physiatry Nurse Practitioner to join our team in Skilled Nursing Facilities. The role partners with a supervising Physiatrist and rehab-focused care for geriatric patients recovering from surgeries or neurological events.
You will provide functional assessments, develop treatment plans, monitor progress, and support discharge planning in a multi-site, in-person setting. Competitive salary with monthly bonus opportunities and comprehensive benefits offered.
Qualifications
- Active and unrestricted Nurse Practitioner license in Illinois.
- Board-certified in Adult, Geriatric, or Family Nurse Practitioner.
- Master’s Degree in Nursing from an accredited program.
- PM&R, geriatrics, internal medicine, or SNF experience preferred.
- Strong functional assessment, pain management, rehab-focused care.
Responsibilities
- Perform comprehensive rehab-focused assessments and mobility goals.
- Develop and update individualized treatment plans with the supervising Physiatrist.
- Conduct regular follow-ups to monitor progress and adjust care.
- Participate in interdisciplinary care meetings and rounds.
- Collaborate with therapy teams on milestones and barriers to progress.
- Educate patients and families on diagnoses, recovery goals, and strategies.
- Document visits and outcomes clearly in the EMR.
- Support discharge planning and transitions to maintain rehab progress.
- Serve as a clinical resource for musculoskeletal and neurologic conditions.
