Job summary

Community Physicians in Bolingbrook, IL is looking for a Nurse Practitioner or Physician Assistant to join our structured Skilled Nursing Facility (SNF) Physiatry Program. This is a full-time position where you will directly support a board-certified physiatrist in providing comprehensive care to post-acute patients.

The ideal candidate will have national certification and experience in physiatry or geriatrics, offering a chance to influence therapy strategies and improve patient outcomes.

Qualifications

• 1-3+ years experience in physiatry, geriatrics, post-acute, or rehabilitation preferred.
• Skilled Nursing Facility experience strongly preferred.
• Ability to work independently in a structured team environment.

Responsibilities

• Participate in structured daily rounding within designated SNFs.
• Provide comprehensive PM&R services for post-acute and long-term care residents.
• Work closely with an experienced physiatrist for mentorship and oversight.
