Job summary

NPHire is seeking a nurse practitioner or physician assistant to round on skilled nursing residents in San Francisco. This 1099 contract role offers a flexible schedule around your primary job, visiting facilities for focused pain check-ins.

Earn per-visit pay with a daily minimum, and benefit from onboarding and tail malpractice coverage. Work involves prescribing pain meds, coordinating therapies, and documenting care in TEBRA and PointClickCare.

Qualifications

• Active, unrestricted California NP or PA license.
• DEA license active; comfortable prescribing controlled substances.
• Travel between facilities across San Francisco.

Responsibilities

• Assess residents’ pain, severity, and response during focused rounds.
• Adjust plans combining medication, non-drug, and interventional approaches.
• Prescribe and manage pain meds, including controlled substances, per regs.
• Coordinate referrals for PT, acupuncture, spinal blocks, and interventional care.
• Guide residents and families on pain management strategies.
• Partner with physicians and facility staff to keep care aligned.
• Document in TEBRA and PointClickCare.

Job description

Join a growing geriatric pain management team in San Francisco as an NP or PA, rounding on skilled nursing residents on your own schedule. You’ll follow the same residents week to week and manage their pain medications, for a population that rarely gets this level of dedicated pain care. This is 1099 contract work built around your primary job: round before clinic, after clinic, on weekends, or on days off. Visits are focused pain check-ins, not full exams, and most providers see 10 to 15 residents an hour and finish in 4 to 5 hours. You earn $10 per visit, with at least $150 guaranteed at every facility you work at in a day. New grads are welcome with structured onboarding, and malpractice with tail coverage is included.

Key Responsibilities

• Assess each resident’s pain, severity, and response to treatment during focused rounds
• Adjust individualized plans combining medication, non-drug, and interventional approaches
• Prescribe and manage pain medications, including controlled substances, per state and federal regulations
• Coordinate referrals for physical therapy, acupuncture, spinal blocks, and interventional care
• Guide residents and families on pain management strategies
• Partner with physicians and facility staff to keep care aligned
• Document in TEBRA and PointClickCare

Qualifications

• Active, unrestricted California NP or PA license
• AGACNP-BC, AGPCNP-BC, FNP-BC, or PA-C certification
• Active DEA | Comfortable prescribing controlled substances
• Able to travel between facilities across San Francisco
• New grads welcome with RN experience in ortho, pain management, SNF, or high-volume ER (county or trauma ER a strong plus)
• TEBRA or PointClickCare experience a plus

Schedule & Compensation

• Guaranteed $150 minimum per facility per day (a floor, not an extra payment)
• ~$90–$150/hr of rounding | ~$400 per typical day | ~$4,000/month at ~400 visits
