Job summary

NPHire is expanding its pain management practice in San Francisco and seeks NPs and PAs to provide focused pain visits for skilled nursing facilities. You will round weekly on residents, adjust treatment plans, and coordinate with facility staff. New grads are welcome with onboarding support.

Rounding typically 4–5 hours per day, with 10–15 residents per hour; you earn per visit with a $150 guaranteed daily minimum. Malpractice with tail coverage is provided, and new facilities open regularly.

Qualifications

• Active, unrestricted California NP or PA license.
• Certifications in AGACNP-BC, AGPCNP-BC, FNP-BC, or PA-C.
• Active DEA and ability to prescribe controlled substances.
• Willingness to travel between facilities across San Francisco.
• RN experience in ortho, pain management, SNF, or high-volume ER for new grads.
• TEBRA or PointClickCare experience a plus.

Responsibilities

• Assess residents' pain, severity, and response during focused rounds.
• Adjust plans combining medication, non-drug, and interventional approaches.
• Prescribe and manage pain medications per regulations.
• Coordinate referrals for PT, acupuncture, spinal blocks, and interventional care.
• Guide residents and families on pain management strategies.
• Partner with physicians and facility staff; track outcomes over time.
• Document in TEBRA and PointClickCare.

Job description

Geriatric pain in skilled nursing facilities is one of the most undertreated areas in medicine. A growing pain management practice in San Francisco is hiring NPs and PAs who want to be the provider that changes that, on a schedule that fits around their main job. You’ll round on the same residents weekly in focused pain visits, not full exams, adjusting treatment and working with facility staff. Most providers see 10 to 15 residents an hour and finish in 4 to 5 hours. You earn $10 per visit, with at least $150 guaranteed at every facility you work at in a day. New grads are welcome with structured onboarding. Malpractice with tail coverage is included, and new seats open as the practice adds facilities.

Key Responsibilities

• Assess each resident’s pain, severity, and response to treatment during focused rounds

• Prescribe and manage pain medications, including controlled substances, per state and federal regulations
• Coordinate referrals for physical therapy, acupuncture, spinal blocks, and interventional care
• Guide residents and families on pain management strategies
• Partner with physicians and facility staff, and track how treatment is working over time
• Document in TEBRA and PointClickCare

Schedule & Compensation

• Guaranteed $150 minimum per facility per day (a floor, not an extra payment)
• ~$90–$150/hr of rounding | ~$400 per typical day | ~$4,000/month at ~400 visits
• Fully flexible: before or after clinic, weekends, or days off
