Job summary

Elderwood Skilled Nursing & Rehabilitation in Burlington, VT seeks a compassionate Occupational Therapist (OT) to join a collaborative, resident-focused rehab team. You will evaluate and treat residents, develop individualized therapy plans, and work with an interdisciplinary group to maximize independence and safety in daily activities.

Ideal candidates have an approved OT degree, state licensure, CPR/BLS, and 2+ years of geriatric/ subacute experience.

Qualifications

• Graduation from an approved school of occupational therapy.
• State license to practice occupational therapy.
• Two years’ experience in occupational therapy preferred.
• Current, valid CPR/BLS credential.

Responsibilities

• Provide skilled occupational therapy evaluations and treatments per physician orders.
• Develop and update individualized treatment plans and care goals.
• Maintain timely and accurate clinical documentation.
• Collaborate with interdisciplinary teams regarding resident progress and discharge planning.
• Participate in resident care planning meetings.
• Support resident and family education initiatives.
• Mentor and provide clinical support as needed.
