Job summary

Premier Rehab & Healthcare at Burlington is actively seeking a dedicated Occupational Therapist (OT) to join our in-house rehabilitation team in Burlington, Vermont. This full-time position focuses on delivering person-centered care, developing individualized treatment plans, and promoting independence and safety for residents and patients.

You will work with an experienced clinical team to provide comprehensive OT evaluations, plan and implement interventions, and monitor progress.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current and valid Vermont OT license.
• Must be in good standing with licensing/regulatory agencies.
• Previous experience in a skilled nursing facility or long-term care setting preferred.
• Strong knowledge of occupational therapy treatment practices and rehabilitation techniques.
• Strong clinical, communication, organizational, and interpersonal skills.
• Ability to work effectively with residents and patients with varying needs.
• Commitment to providing compassionate, patient-centered care.
• Familiarity with electronic therapy documentation systems, including Net Health, preferred.

Responsibilities

• Perform comprehensive OT evaluations and interpret findings.
• Develop individualized treatment plans based on physician orders and patient needs.
• Provide skilled OT interventions to improve independence and quality of life.
• Address ADLs, mobility, strength, coordination, cognition, balance, and other areas of function.
• Monitor progress and modify treatment approaches as needed.
• Set and update individualized treatment goals.
• Educate residents, patients, families, and caregivers on plans and safety.
• Collaborate with physicians, nursing, therapy team, and other staff.
• Supervise and guide OT Assistants per regulations and standards.
• Communicate changes in patient condition and progress to the team.
• Document evaluations, treatments, progress, goals, and discharge planning.
• Participate in required meetings and duties as directed by Rehabilitation leadership.
