Job summary

Premier Rehab & Healthcare at Burlington in Burlington, Vermont, seeks a dedicated Full-Time Occupational Therapist to join our in-house rehabilitation team. You will evaluate residents, develop individualized treatment plans, and promote independence, safety, mobility and quality of life.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current and valid Vermont Occupational Therapist license.
• Experience in skilled nursing facility or long-term care preferred.
• Strong clinical and communication skills; ability to work with diverse patient needs.
• Familiarity with electronic documentation systems (Net Health) preferred.

Responsibilities

• Perform comprehensive occupational therapy evaluations and interpret findings.
• Develop individualized treatment plans based on physician orders and patient needs.
• Provide skilled interventions to improve independence and quality of life.
• Address ADLs, mobility, strength, cognition, balance and other performance areas.
• Monitor progress and adjust treatment approaches as needed.
• Educate residents, patients, families, and caregivers on plans and safety.
• Collaborate with physicians, nursing, and therapy team members.
• Supervise and guide Occupational Therapy Assistants per regulations.
• Document evaluations, treatments, progress, goals, and discharge planning.

Occupational Therapist (OT)

Premier Rehab & Healthcare at Burlington has an exciting opportunity for a Full-Time Occupational Therapist (OT) to join our in-house rehabilitation team in Burlington, Vermont.

We are seeking a dedicated and compassionate Occupational Therapist to provide high-quality, person-centered rehabilitation services to our residents and patients. You will work alongside an experienced clinical team to develop individualized treatment plans that promote independence, safety, mobility, and quality of life.

Occupational Therapist Responsibilities

• Perform comprehensive occupational therapy evaluations and interpret evaluation findings.

• Develop individualized treatment plans based on physician orders, clinical findings, and patient needs.

• Provide skilled occupational therapy interventions to improve functional independence and quality of life.

• Address activities of daily living (ADLs), functional mobility, strength, coordination, cognition, balance, and other areas of occupational performance.

• Monitor patient progress and modify treatment approaches as clinically appropriate.

• Establish and update individualized treatment goals based on patient progress and clinical needs.

• Educate residents, patients, families, and caregivers on treatment plans, adaptive techniques, safety, and strategies to promote independence.

• Collaborate with physicians, nursing staff, therapy team members, and other members of the interdisciplinary team.

• Supervise and provide guidance to Occupational Therapy Assistants in accordance with applicable regulations and professional standards.

• Communicate changes in patient condition, progress, and treatment needs to the appropriate healthcare team members.

• Complete accurate and timely documentation of evaluations, treatments, progress, goals, and discharge planning.

• Participate in required meetings and other duties as assigned by the Director of Rehabilitation.

Occupational Therapist Qualifications

• Graduate of an accredited Occupational Therapy program.

• Current and valid Occupational Therapist license in Vermont.

• Must be in good standing with applicable licensing and regulatory agencies.

• Previous experience in a skilled nursing facility (SNF) or long-term care setting preferred.

• Strong knowledge of occupational therapy treatment practices and rehabilitation techniques.

• Strong clinical, communication, organizational, and interpersonal skills.

• Ability to work effectively with residents and patients with a variety of needs and abilities.

• Commitment to providing compassionate, patient-centered care.

• Familiarity with electronic therapy documentation systems, including Net Health, preferred.
