Job summary

Oasis Therapy Services seeks a licensed Physical Therapist to provide one-on-one rehabilitation at Eden Vista Litchfield in Litchfield, Minnesota. The PT will evaluate residents, develop individualized plans, and collaborate with the care team to ensure optimal outcomes.

Role emphasizes one-on-one treatment, ongoing progress monitoring, and discharge planning. CPR certification and CAPTE-accredited education are required, with geriatrics experience preferred.

Qualifications

• Licensed physical therapist in Minnesota (CAPTE-accredited program graduate).
• Current CPR certification.
• Experience in geriatric or senior living settings preferred.

Responsibilities

• Evaluate residents and develop individualized physical therapy plans.
• Provide one-on-one treatment focused on strength, balance, gait, transfers, mobility, pain management, and fall prevention.
• Monitor residents' progress and modify treatment plans as needed.
• Complete discharge planning and provide recommendations for continued care.
• Educate residents and caregivers on exercises, mobility strategies, and equipment.
• Direct and supervise Physical Therapist Assistants as appropriate.
• Complete accurate Net Health documentation and collaborate with the care team.
