Job summary

Oasis Therapy Services in Litchfield, Minnesota seeks a licensed Speech-Language Pathologist (SLP) to provide one-on-one rehabilitation in independent living, assisted living and memory care settings. This PRN role focuses on speech, language, cognitive-communication and swallowing therapy to support residents' safety, independence, and quality of life.

Qualifications

• Active Minnesota SLP license for independent practice.
• Master's or doctoral degree in Speech-Language Pathology.
• ASHA CCC-SLP preferred.
• Current CPR certification.
• Strong clinical assessment, documentation and organizational skills.
• Experience in geriatric rehabilitation, memory care, or related settings preferred.

Responsibilities

• Conduct comprehensive SLPS evaluations across speech, language, voice, cognitive-communication and swallowing domains.
• Develop and implement individualized treatment plans.
• Provide one-on-one SLPS interventions including cognitive-communication therapy and dysphagia management.
• Evaluate swallowing difficulties and advise safe dietary modifications in coordination with the care team.
• Provide cognitive-communication interventions for residents with dementia and memory loss.
• Monitor progress, adjust plans and complete reassessments and discharge planning.
• Educate residents, families and caregivers on communication strategies and home exercises.
• Collaborate with therapists, nursing, physicians and other staff to coordinate resident care.
• Complete evaluations, progress notes and discharge summaries in Net Health.
